Student Registration

Required items at the time of registration:

•Printed copy of all student registration forms downloaded from this website. These forms are also available at the buildings.

•Copy of certified birth certificate (not a hospital certificate)

•Verification of Residency document. (acceptable items )

•Moving into the District or building a new home 
New Residency Requirement.

•Most recent proof of custody if the child(ren) are not living with both natural parents.

•Child's Social Security Card/ Number.

•Current Report Card or transcript information.

•Child's immunization records (Click here to view list of required vaccinations)

If you do not have computer access for online registration, please call (937) 289-2471 ext. 1402 to setup a time to complete the forms on our campus.
